Who is i-go for?

You can join i-go if you:

• 0–25 years old
• Live in East Sussex, or
• Study in East Sussex

And if you have any of the following:

• Education Health and Care Plan.
• Additional Needs Plan. (This could also be known as Specific Support Plan or Provision Map)
• Disability Living Allowance. (DLA)
• Personal Independence Payment. (PIP)
• Blue Badge.
• Concessionary Disabled Person’s Bus Pass.
• Medical Diagnoses. (Including a diagnosed Mental Health Condition)
• No Medical Diagnosis (including Social, Emotional, Mental Health Conditions) *
  *An application support form must be completed if you are applying without a medical diagnosis

Application Support Form*

A Support form is needed when you or your child does not have a medical diagnosis. A health, education or social care professional must sign an application support form.

We aim to issue your card within 2 to 4 weeks.
The card will need to be renewed at 4, 11 and 16 years old. We will send you a reminder near the time.
